We have an exciting opportunity for a HICCS Community Care Assistant to join an organisation making a real difference in people’s lives.
We're looking for a committed, reliable and experienced care support worker to join our expanding Home Independence and Crisis Support Service (HICSS).
Week 1 - Monday, Saturday & Sunday 7pm - 7am.
Week 2 - Thursday & Friday 7pm - 7am.
You`ll be supporting customers being discharged from hospital and returning home by ensuring the right care and support is provided during this period of transition including completing assessments, crisis interventions and emergency response, personal care, medication, supporting with use of daily living/rehabilitation aids.
Applicants must have a full driving license and access to own vehicle as you will be expected to work across all areas of South Tyneside.
