Humana is seeking a Remote RN Care Manager to support members with chronic health conditions. Responsibilities include managing care strategies and ensuring optimal outcomes for patients through telephonic support.
The ideal candidate will be bilingual in English and Spanish, possess a BSN degree, and have an active RN license in both Puerto Rico and Florida. This is a full-time position with a pay range of $56,900 - $78,200 per year plus bonuses.
We are seeking compassionate and motivated Registered Nurses to join our team in Puerto Rico as a RN Care Manager. This is a remote, Work-At-Home position, but you must live in Puerto Rico to be considered.
You will report directly to a Manager of Care Management and support our CarePlus and Humana members with chronic and complex health conditions through telephonic care management. You will build strong relationships with members through communication and engagement and use your clinical knowledge to assess and evaluate their needs to achieve or maintain an optimal wellness state. We are looking for bilingual RNs who are proficient in both Spanish and English. Success in the role requires a strong knowledge of chronic condition management including treatment plans, pharmacological treatment, and signs and symptoms.
Please be advised, our screening and interview process is conducted entirely in English, and if selected for this position, you will be required to pass language proficiency tests in both English and Spanish before we can extend an offer.

Language Proficiency Testing
Any Humana associate who speaks with a member in a language other than Spanish must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
Travel: While this is a remote position, occasional travel to Humana’s offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$56,900 - $78,200 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
