Seeking Physician Assistants and Nurse Practitioners who are passionate about community health and family medicine to perform Annual Health Assessments, in Fresno and surrounding areas of California's Central Valley.

This is a full-time, in-clinic position serving underserved populations in modern facilities. Work weeks are Monday to Friday, 8-hour days, with comprehensive compensation and benefits.

Responsabilidades

• Conduct Annual Health Assessments including evaluation of history, systems review, med reconciliation, vitals, and physical exam
• Perform evidence-based screenings including point-of-care testing as appropriate
• Identify diagnoses for care management and medical management
• Formulate current/past medical conditions from assessment findings
• Communicate assessment findings to inform the patient's primary care provider
• Identify urgent/emergent situations and intervene appropriately
• Educate patients on disease processes, medications, and adherence

Responsibilities

• Conduct Annual Health Assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
• Perform evidence-based practice screenings including point of care testing (as appropriate)
• Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
• Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of the assessment
• Communicate findings in the assessment that will be used to inform patients primary care provider of potential gaps in care
• Identify urgent and emergent situations and intervene appropriately
• Educate patients on topics such as disease process, medication, and compliance

Compensation/Benefits

• Salary starts at $137k/year plus bonus
