Britt Medical is seeking a Family Nurse Practitioner to join a PACE team delivering comprehensive, longitudinal care to the elderly within a geriatric primary care model. The APP will evaluate, manage chronic and acute conditions, coordinate care plans, and support safe transitions of care alongside an interdisciplinary team.

This full-time, in-clinic role emphasizes thoughtful clinical decision making, high-quality patient outcomes, and working with underserved populations in modern facilities.

Formación

• Licensed as a Family Nurse Practitioner in California.
• Current DEA registration.
• BLS or ACLS certification.
• Graduate of an accredited Family Nurse Practitioner program.
• National certification in primary care.
• Certified to practice in California state.
• Experience in health centers or geriatric care preferred.

Responsabilidades

Seeking Family Nurse Practitioner passionate about community health and family medicine to join PACE (Program of ALL Inclusive Care for the Elderly) team.

This Primary Care APP role delivers comprehensive, longitudinal care to a geriatric population within a PACE (Program of All Inclusive Care for the Elderly) model. the provider works closely with an interdisciplinary team (IDT) to assess, plan, and manage care, ensuring continuity across all care settings.

The APP is responsible for performing detailed evaluations, managing, chronic and acute conditions, coordinating care plans, and supporting safe transitions of care. This role emphasizes thoughtful clinical decision making, minimizing unnecessary testing and referrals while maintaining high quality, patient centered outcomes.

• Full-time position, in-clinic only
• Work with underserved and culturally diverse patient populations
• Modern facilities with state-of-the-art equipment
• Monday-Friday, 8 hour days

Qualifications

• Certified: California Family Nurse Practitioner License
• DEA Certificate
• BLS or ACLS certification
• Graduate of an accredited Family Nurse Practitioner Program
• National certification in his/her primary care specialty
• Qualified to practice by certification in the State of California
• Experienced FNP (particularly those who have worked in Health Centers) in primary care or geriatric care are preferred.

Compensation/Benefits

• Salary starts at $137k/year plus bonus
