Job summary

Dunes Hospice is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings. The role emphasizes using community resources to meet patient and family needs and deliver education and compassionate care.

Qualifications

• Registered Nurse with current Indiana licensure.
• Current CPR certification required.
• Minimum two years clinical experience; hospice care experience preferred but not required.
• Must be a licensed driver with insured vehicle; driving is required.
• Strong observation, verbal and written communication skills; problem solving and basic math skills.

Responsibilities

• Perform initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provide professional nursing care following the nursing process and physician's orders.
• Develop and revise plans of care with patient/family input; implement preventive and rehabilitative nursing actions.
• Coordinate care with physician and interdisciplinary team; ensure safe care environment.
• Participate in on-call duties and ensure availability of equipment and services.
• Document accurate clinical notes and report changes in patient status to the physician.
