Job summary

Applitrack in Minnesota is seeking a compassionate Licensed Practical Nurse (LPN) to provide health support to students across multiple school buildings. Assignments will be based on student health needs, medical needs, staffing demands, and district priorities.

The LPN will work under the direction and delegation of a Licensed School Nurse and collaborate with school staff, families, and healthcare providers to help students safely access their educational programs.

Qualifications

Current Minnesota LPN license in good standing.
Current CPR certification.
Ability to provide nursing care to children and adolescents with a range of health needs.
Ability to respond calmly during medical emergencies.
Strong communication, organization, documentation, and collaboration skills.
Ability to travel between district buildings during the workday.
Valid driver’s license and reliable transportation.

Responsibilities

Provide direct nursing care to students according to individualized health plans and district procedures.
Administer medications and nursing procedures under the direction of a Licensed School Nurse.
Support students with chronic, acute, or complex health conditions during the school day.
Monitor health status and promptly communicate concerns to the Licensed School Nurse and other staff.
Respond to student illnesses, injuries, and medical emergencies.
Maintain accurate, timely, and confidential health documentation.
Communicate professionally with families, school staff, healthcare providers, and the student support team.
Travel between district buildings and adjust assignments as needed.
