Job summary

Position Summary

Reporting to the Youth Connections Team Leader, the Youth Transition Coordinator is responsible for providing holistic, relationship-based prevention support to youth and young adults in Member Communities as they prepare for adulthood and greater independence. As an integral member of a multidisciplinary team, the Youth Transition Coordinator collaborates with young people to develop individualized Transition Plans, build practical life skills, strengthen self-advocacy, connect to culture and community, and address goals related to education, employment, housing readiness, health and wellness, financial literacy, and independent living. The role combines one-to-one support with workshops, groups, events, cultural learning opportunities, and community-based programming.

As a member of the SCFSS organization, the Youth Transition Coordinator is committed to the collective response to the Call to Action through the four Strategic Directions and supports a pathway of change to exercise nłeʔkepmx and syilx inherent rights. In particular, the Youth Transition Coordinator champions core elements of the strategic goal of ‘Protection to Prevention’.

OVERVIEW OF DUTIES & RESPONSIBILITIES

• Develop and maintain individualized Transition Plans that support youth in achieving independence and personal goals.
• Provide mentorship, advocacy, and ongoing support to youth transitioning into adulthood.
• Support life-skills development in areas such as budgeting, household management, employment readiness, communication, and healthy relationships.
• Connect youth with education, employment, housing, health, wellness, and community resources.
• Facilitate workshops, events, and activities that promote life skills, leadership, wellness, and personal growth.
• Foster meaningful relationships with youth and support the development of strong personal and community support networks.
• Promote cultural connection by facilitating access to nłeʔkepmx and syilx teachings, traditions, Elders, and community activities.
• Collaborate with internal teams, community partners, and service providers to coordinate supports and services.
• Maintain accurate, timely, and confidential documentation, records, reports, and case notes.
• Monitor youth progress, celebrate achievements, and support transition, graduation, and closure planning.
• Participate in team meetings, collaborative case planning, and ongoing professional development.
• Ensure services are delivered in accordance with organizational policies, FOIPPA, OH&S requirements, trauma-informed practice, and applicable legislation.
• Promote The Society's Protection to Prevention approach by empowering youth, strengthening natural supports, and fostering long-term wellness and self-determination.

EDUCATION AND EXPERIENCE

• Diploma, certificate, or degree in Social Work, Social Services, Child and Youth Care, Human Services, Community Support, Indigenous Studies, Health Care Assistance, Education, Psychology, Sociology, or a related field; or an equivalent combination of education, training, lived experience, and employment experience.
• Minimum two (2) years' experience supporting youth, young adults, families, or vulnerable populations in community, social service, education, health care, cultural, housing, employment, or child welfare settings.
  
• Applicants with a Health Care Assistant (HCA), Registered Care Aide (RCA), Community Support Worker, or equivalent credential and relevant experience are encouraged to apply.
• Strong advocacy skills, with the ability to handle complex and emotionally challenging situations
• Ability to maintain confidentiality, diplomacy and objectivity when communicating and interacting with young people, families, colleagues, leaders, and community
• Proficiency in Microsoft Office (Excel, Word, PowerPoint) and various databases, search engines and Outlook/email
• Demonstrated knowledge and professional and personal practice of nłeʔkepmx and syilx traditional values, cultural practice, and protocols is an asset
  

JOB REQUIREMENTS

• Clear Criminal Record Check and Ministry of Child and Family Development Prior Contact Check required before commencement of employment
• Valid Class 5 BC driver’s license and acceptable driver’s abstract required
