Youth Stabilization & Housing Support Specialist

Job summary

360°kids is a vital community agency serving York Region. We seek a Child and Youth Worker to provide supervision, support, and housing assistance to youth emerging from crisis within the connections program.

In this role you will conduct intake, coordinate referrals to housing and counseling services, maintain daily records in HIFIS, and participate in weekly resident meetings. Evening overnight shifts are required Sun-Thu.

Qualifications

• Post-Secondary degree/diploma in a Child/youth related field or the equivalent
• Experience with youth and at-risk populations is strongly preferred
• Experience using a case management system or HIFIS and VI-SPDAT an asset
• Therapeutic Crisis Intervention (TCI) and Suicide Prevention training an asset
• Current First Aid/CPR certification an asset
• Current Broad Record Check (BRC)
• Knowledge of CFSA and community resources
• Excellent communication and client-focused counselling skills
• Ability to adapt with a flexible approach and strong teamwork
• Shift timings: Sun-Thursday Overnight rotation between 11pm-7am and 11:59pm-8am

Responsibilities

• Follow up with youth regarding case plans and goals.
• Assess housing needs and assist with housing search.
• Make referrals to Housing, Counselling, Employment, Education and other services.
• Provide crisis intervention and conflict resolution as required.
• Conduct intake using VISPDAT and related tools within 72 hours of admission.
• Maintain daily documentation in HIFIS and client records.
• Lead weekly resident meetings and address program concerns.
