Job summary

Northern Health is seeking a Social Worker to support youth and families in mental health and substance use treatments within the Specialized Services department. The role involves community outreach, assessments, early interventions, and referrals to help youth stay connected to care.

You will work within a multidisciplinary team, delivering crisis response, discharge planning, and seamless transitions between services, with a focus on harm reduction and client-centered care.

Qualifications

• Bachelor's Degree in Social Work from an accredited university.
• Two (2) years' recent, related experience with youth mental health/substance use.
• Current full registration with the BC College of Social Workers.
• Valid BC Drivers' license; use of personal vehicle may be required.

Responsibilities

• Provide assessments, early interventions and referrals for youth with mental health and substance use issues.
• Deliver community-based crisis response and treatment planning.
• Collaborate with multidisciplinary health care teams for discharge planning.
• Provide outreach in school, street-based and community settings.
• Promote harm-reduction approaches in collaboration with partners.
