Job summary

TADH is seeking a Wound Care Nurse (RN) in Ontario to support wound care across the organization. The role includes assessment, plan development, treatment initiation, and education for staff, with leadership in wound care programs.

The successful candidate will be a CNS/OCN with strong collaboration skills, able to coordinate with physicians and care teams, and participate in program development and quality improvement initiatives.

Qualifications

• Minimum 3 years of medical-surgical nursing experience.
• Registered with the College of Nurses of Ontario.
• CAN/WOWCC IIWCC or WOCC(C) certification preferred or willingness to obtain.
• Strong evidence-based practice and protocol literacy.

Job description

Reporting to the Surgical Program Director, and working collaboratively in the wound program with the clinical team, and patients, the wound nurse is a role implemented to support wound care across the organization. The nurse is responsible for providing support, treatment recommendations and follow-up care on identified complex/problematic wounds and ostomies. Functions of the role may include consultation, direct patient care, and education and mentorship. The nurse is responsible to ensure best practice standards are followed and best possible wound care is provided to patients across the organization.

Position Responsibilities

• Perform a detailed holistic assessment of clients with wound, ostomy or continence related concerns.
• Formulate treatment recommendations in collaboration with the interprofessional team, and develop a plan of care
• Initiate and formulate treatment plan for wound patients
• Provide education and mentorship to team members
• Provide wound consultation to care providers within the organization
• Demonstrate knowledge and understanding of common wound etiology and wound standard of care
• Direct and evaluate nursing care provided to patients with wounds
• Coordinate and lead weekly wound team rounds-measurements, care plan reviews, treatment recommendations, physician orders, required documentation
• Evaluate supply needs and update as required
• Monitor changes in regulations and best practice standards
• Perform yearly wound prevalence across the Surgical, Medical programs
• Review and update wound program, protocols and related documents
• Review wound care related incidents and provide suggestion/recommendations

Position Requirements

• Minimum of 3-year medical surgical nursing experience
• BScN, or Nursing Diploma with relevant experience and advanced education in wound care
• Registered with the College of Nurses of Ontario
• Experience and comfort with meeting and building relationships with clinical teams
• Strong understanding of evidence-based practice
• Ability to read and understand clinical protocols
• Comprehensive knowledge of standards of practice and professional guidelines set forth by the College of Nurses and Nurses Specialized in Wound, Ostomy and Continence Canada (NSWOCC), International Interprofessional Wound Care Course (IIWCC)
• CAN certification credential WOCC(C) Wound, ostomy, continence or International Interprofessional Wound Care Course (IIWCC)
• certified preferred-or willingness to obtain certification
• A willingness to participate in further education programs/courses to remain current (Yearly Wounds Canada Institute Conference)
• Proven ability for self-directed leadership and program development
• Demonstrated ability to develop program-print and computer-based resources and program data collection
• Project management and health care education teaching experience
• Strong oral and written skills, including the ability to create and deliver effective presentations
• Proven strength in working in a collaborative model with the interdisciplinary team
• Technologically literate with strong computer skills including familiarity with Microsoft Word,
• Excellent interpersonal skills, including the ability to facilitate communication between primary care physicians, specialists, community partners, and the health care team; and the capacity to professionally interact with individuals in person and by telephone, facilitating a positive work culture
• Advanced organizational skills and time management skills
• Ability to work in a busy environment with concurrent demands and tight deadlines.
• Must maintain regular attendance at work.
• Must have the ability to meet the physical demands of the role.
• Demonstrates accuracy and thoroughness
• Supports processes of quality improvement and program planning
• Bilingualism will be considered an asset.
• Starting salary of $41.15/hour; $80,242.50/year (As per Step 1 of the ONA collective agreement.) range to 58.98/hour; $115,011.00/year (As per Step 8 of the ONA collective agreement.)
