Job summary

WNHAC seeks a Homecare Nurse to deliver primary health care in client homes across specified communities in Ontario. You will perform assessments, nursing diagnoses, and treatment within the nursing scope of practice while collaborating with a professional team to promote health and prevent disease.

The role emphasizes wholistic care, Indigenous cultural understanding, immunizations, and travel to client homes.

Qualifications

• Currently registered as a Registered Nurse with the College of Nurses of Ontario.
• Willingness to work to a full scope including delegated acts via medical directives.
• Experience in home care nursing is an asset.
• Immunization knowledge and vaccine administration is an asset.
• Experience in Indigenous settings and trauma-informed, client-centered care is valued.

Responsibilities

Homecare Nurse

We provide primary health care through a range of options including prevention, assessment, diagnosis and treatment. Our interprofessional teams include health professionals, educators and support workers representing both traditional Indigenous and contemporary models of care and using wholistic health and community development concepts.

The Home Care Nurse (HCN) works within an interprofessional team comprised of registered and non-registered healthcare professionals, both directly and under the facilitation of medical directives, to facilitate the coordination of client care and supports optional functioning of the team to provide wholistic healthcare services at the community level. The HCN provides primary health care services in the client home environment in specified communities, with an emphasis on health promotion and disease prevention within the professional scope of practice established by the College of Nurses of Ontario.

NOTE: Traditional practices involve cultural ceremonies and burning sacred medicines including tobacco, sweetgrass, sage and cedar within the work setting.

PRIMARY DUTIES / RESPONSIBILITIES:

• Carry out assessment, nursing diagnosis, and treatment within the nursing scope of practice within the client’s home environment, including but not limited to assessing episodic issues, chronic disease monitoring, age[1]related screening, wound care, immunizations, client education.
• Collaborate with professional team members to ensure comprehensive wholistic care for clients, making referrals and seeking and/or providing consulting support as appropriate.
• Work with clients to complete the initial intake assessment, development of client care plan, and monitoring and evaluation care plans to promote overall health and wellness.
• Support to the client intake process through completion of medical history data entry into CPP, including immunization records.
• Collaboration with the Community Health Nurse to support delivery of mandatory and/or discretionary community health programs in the client home environment (ex. Seasonal immunization programs).
• Maintain current and accurate medical records, reports, and other documentation to ensure comprehensive, consistent care and services, and to meet all accountability requirements.
• Active participation in organizational teams and committees.
• Other duties as required. In extenuating situations or in the event of an emergency, may be redeployed to ensure provision of essential services.

QUALIFICATIONS:

• current registration as a Registered Nurse with the College of Nurses of Ontario;
• willingness to work to a full scope including performing delegated acts through medical directives;
• experience in home care nursing a definite asset;
• training and experience in basic and/or advanced wound care an asset;
• working knowledge of provincial immunization schedule and experience with the administration of vaccines an asset;
• experience and demonstrated ability to work in a collaborative, de-colonial, trauma-informed, and client-centered approach to support WNHAC values in meaningful ways;
• deep understanding and experience of Indigenous culture, values, and perspectives as well as traditional and contemporary health care practices, and wholistic health and wellbeing;
• ability to speak and understand Anishinaabemowin a definite asset;
• knowledge of historical trauma and the impacts of that history on the health of people, families, and communities;
• experience working in an Indigenous organization; understanding and/or willingness to learn of Indigenous culture and values;
• proof of immunization in compliance with policy requirements is mandatory;
• valid drivers’ license, own vehicle and willingness and ability to travel required;
• criminal records check and current drivers’ abstract required

Salary: $38.48-$51.89 per hour with excellent benefits, including HOOPP pension plan. Salary is in accordance with the Ontario Nurses’ Association Contract (ONA).
