Job summary

Oak Valley Health in Ontario is seeking a Registered Practical Nurse (CUPE) for Transitional Care to work with an interprofessional team delivering patient/family centred care for alternative level of care patients awaiting transition back to the community.

The role requires a current Certificate of Registration with the College of Nurses, current BLS, experience with aggressive behaviours, solid clinical skills in inpatient rehab or medical/surgical care, and strong communication, organization

Qualifications

• Current Certificate of Registration with the College of Nurses.
• Current BLS certification.
• Experience managing patients with aggressive behaviours.
• Inpatient rehabilitation and/or inpatient medical/surgical experience with strong clinical skills.
• RNAO membership is preferred.
• Excellent physical assessment skills.
• Proficient computer skills.
• Demonstrated commitment to patient/family centred care and interprofessional teamwork.
• Strong organization, communication and teaching abilities.
• Evidence of ongoing professional development and leadership experience.

Responsibilities

• Work closely with an interprofessional team to provide care for ALC patients awaiting transition back to the community.
• Deliver patient/family centred care in a safety focused environment.
• Demonstrate critical thinking and problem solving within the team.
