Job summary

CBI Home Health in Alberta is seeking a Therapy Practice Lead to guide therapy teams and advance integrated care across home health services. You will balance a clinical caseload with leadership duties, ensuring high-quality patient outcomes and adherence to standards.

The role requires supervisory experience, professional registration, and a valid Alberta driver’s license. You’ll collaborate with care staff to mentor teams, optimize operations, and drive continuous improvement while supporting

Qualifications

  • Current registration with the applicable regulatory body (RD/PT/OT/SLP/SW) in good standing.
  • At least 2 years of supervisory experience and/or home health service delivery experience.
  • Ability to establish rapport with clients and care staff.
  • Valid Alberta driver’s license with access to a reliable vehicle.
  • Ability to apply policies and uphold best practices; work independently and as part of a team.
  • Self-motivated with commitment to professional development.

  • Identify opportunities for continuous improvement and drive clinical excellence across service delivery.
  • Participate in recruitment, workforce planning, and staffing coordination for provider coverage.

What You Will Do
  • Guide and support therapy teams in adopting integrated care practices, helping shift practice from discipline-specific service delivery toward collaborative, team-based care with shared goals, coordinated interventions, and a strong focus on patient outcomes.
  • Lead service delivery excellence by overseeing patient care quality, resolving client concerns, managing risk, and ensuring compliance with organizational, regulatory, and professional standards.
  • Carry a clinical caseload
  • Coach, mentor, and support clinical staff through onboarding, performance management, training, and ongoing professional development.
  • Monitor team performance, engagement, retention, and utilization while ensuring staff are aligned with role expectations and equipped to deliver high-quality care.
  • Support clinical operations through caseload management, chart reviews, audits, incident investigations, documentation oversight, and quality assurance activities.
  • Provide clinical guidance, problem-solving support, and education to therapy and care teams, promoting best practices and evidence-based interventions.
  • Identify opportunities for continuous improvement and drive operational and clinical excellence across service delivery.
  • Participate in recruitment, workforce planning, and staffing coordination to ensure appropriate provider coverage and a positive patient experience.
  • Represent the organization with external stakeholders and contribute to initiatives that enhance quality, collaboration, and healthcare outcomes.
What You Will Need To Be Successful
  • Possess current registration and be in good standing with the applicable professional regulatory body (Registered Dietitian, Physiotherapist, Occupational Therapist, Speech-Language Pathologist, or Social Worker).
  • Have 2 years of supervisory experience and/or experience in the home health service delivery
  • Be able to establish and build rapport with clients and Care Staff effectively
  • Possess a valid Alberta driver's license with access to a reliable vehicle
  • Have the ability to provide the best possible solutions to problems that align with policies and uphold best practices
  • Be self-motivated with an ability to work both independently and as an effective member of a team
