Job summary

Qualifications

  • Excellent organizational skills and ability to manage time-sensitive visits and maintain records.
  • Proficiency with Microsoft Office Suite and scheduling/documentation systems.
  • Valid Alberta driver’s license and reliable vehicle.
  • Clinical skills in assessment, care planning, and medication reconciliation.
  • Problem-solving, analytical thinking, and clear communication with diverse stakeholders.
  • Commitment to high-quality, patient-centered care.

  • Lead accurate clinical documentation and ensure regulatory compliance.
  • Champion innovative, client-centered care models to reduce hospital visits and improve quality.

What you will do:

  • The Primary Nurse Clinician provides expert clinical leadership and delivers high-quality, client-centered care to individuals with complex needs in the community.
  • This role supports safe, timely care delivery while strengthening team capability and care continuity.
  • Deliver advanced nursing care for complex patients, including urgent, time-sensitive visits and care plan adjustments.
  • Conduct comprehensive, evidence-based assessments and develop individualized care plans in collaboration with clients, families, and interdisciplinary teams.
  • Mentor, support, and supervise team members to enhance clinical skills, confidence, and professional growth.
  • Lead accurate, timely clinical documentation and ensure compliance with regulatory and organizational standards.
  • Champion innovative, client-centered care models that support hospital avoidance, safe transitions, and continuous quality improvement.

What you will need to be successful:

  • Current registration with the applicable provincial regulatory body.
  • A minimum of 3-5 years of supervisory experience or equivalent clinical leadership experience in home and community healthcare.
  • Advanced Clinical Expertise: Demonstrated ability to deliver acute and advanced care in the home or community setting.
  • Strong Leadership Skills: Proven track record in team development, supervision, and fostering collaboration.
  • Exceptional Organizational Skills: Capacity to manage time-sensitive visits, prioritize patient needs, and maintain comprehensive records.
  • Commitment to Quality Care: Passion for delivering high-quality, safe, and patient-centered services.
  • Strong proficiency with Microsoft Office Suite and experience with scheduling/documentation systems.
  • A valid Alberta driver's license and access to a reliable, insured vehicle.
  • Clinical Expertise: Strong assessment, care planning, and medication reconciliation skills.
  • Problem Solving: Analytical and critical thinking to identify service gaps and implement improvements.
  • Communication: Excellent verbal and written skills for interacting with diverse stakeholders.
  • Organizational: Superior time management and attention to detail for managing competing priorities.
  • Team Collaboration: Commitment to fostering interdisciplinary teamwork and patient-centered care.
