The Integrated Community Access Program is seeking a full-time Community Mental Health Registered Practical Nurse to provide mental health nursing services to individuals with serious mental illness, particularly those diagnosed with psychosis.

As a key member of the outreach team, you will provide clinical assessment and treatment, intensive case management, consultative and educational services to clients, families, and community providers. You will demonstrate initiative and creative approaches to challenging situations and will work collaboratively as this program continues to strive towards specialized evidence-based services in a learning environment. The successful candidate must be registered with the College of Nurses of Ontario in good standing.

Minimum Requirements:
  • Demonstrated competence within the full scope of practice of a Registered Practical Nurse in the provision of assessment, care, medications, interventions, and documentation of care needs.
  • Current CPR-HCP certification.
  • Knowledge and experience working in an outpatient setting with individuals experiencing acute or chronic severe mental illness, including psychosis or psychotic disorders.
  • 3-5 years of recent psychiatric nursing experience in a mental health facility or service.
  • Community mental health service provision experience.
  • Ability to work effectively as part of an interdisciplinary team.
  • Demonstrated knowledge of Canadian Standards for Psychiatric-Mental Health Nursing, and relevant policies and legislation, such as the Mental Health Act and PHIPA.
  • Maintain reflective practice standards as described by the CNO.
  • Knowledge and experience of the challenges involved in community reintegration of long-stay psychiatric inpatients.
  • Demonstrated advanced knowledge of psychotropic medication.
  • Current driver’s license and access to a car.
  • Ability to perform the physical demands of the position.
  • Training and/or experience in facilitating psychoeducational groups.
  • CNA certification in psychiatric/mental health nursing is an asset.
  • Demonstrated excellent interpersonal and effective oral/written communication skills.
  • Ability to work with clients of diverse ethno-racial and cultural backgrounds; proficiency in a second language is considered an asset.
  • Meditech experience preferred.
  • Proven good attendance record with the capability of maintaining this same standard.
