Job summary

Halton Healthcare seeks a Team Lead to oversee day-to-day clinical operations, patient flow, and program growth for Central Intake's Orthopedic and Ophthalmology Hubs across Oakville, Milton, and Georgetown. You will coordinate referral management, support clerical staff, collaborate with stakeholders, and drive quality improvement initiatives while ensuring timely, high-quality patient care.

Travel between sites is required 2–5 times per month; a healthcare degree and clinical leadership are

Qualifications

  • Bachelor’s degree from a healthcare program.
  • Regulated Healthcare Professional preferred.
  • 5–7 years clinical experience in surgical orthopedic, ophthalmology, and ambulatory settings.
  • 3–5 years leading quality improvement or redesign initiatives.
  • Experience coaching and mentoring staff.
  • Ability to travel between sites (2–5 times per month).

Job Description

The Team Lead is responsible for the day-to-day clinical operations, patient flow, and program growth initiatives that support Central Intake for both the Orthopedic and Ophthalmology Hubs.



  • Acting as the point of contact for referral management including coordinating referral flow, liaising with the Central Intake Program, referral sources, and external partner organizations.

  • Supporting clerical staff in referral prioritization and triage activities to ensure appropriate referral routing and efficient operational workflows.

  • Collaborate with internal and external stakeholders to ensure appropriate referral routing and efficient operational workflows.

  • Promote quality improvement and evidence-based practice across the program, supporting staff in delivering high-quality patient care.

  • Lead program growth and development initiatives in partnership with Central Intake team members, ensuring alignment with provincial central intake models and strategic objectives.

  • Foster a collaborative inclusive team environment that encourages clinical inquiry, innovation, and constructive problem-solving.

  • Act as the first point of contact for patient, referral source, and stakeholder concerns, as well as incident investigations, ensuring concerns are addressed promptly and appropriately.

  • Provide the clinical lens to support program growth and operational planning.

  • Provide coaching, education, mentoring, and support to ensure Central Intake team members aligns with standards, program goals, and organizational expectations.


Qualifications

  • Bachelor’s degree from a healthcare program

  • Regulated Healthcare Professional (Registered Nurse, Social Work, Occupational Therapy, Physiotherapy) preferred

  • Minimum of 5-7 years of progressive clinical experience in surgical orthopedic, ophthalmology, and ambulatory healthcare settings.

  • Minimum of 3-5 years participating and/or leading clinical quality improvement projects, patient safety, or service redesign initiatives with demonstrated outcomes.

  • Demonstrated experience managing referral pathways, patient flow, intake processes, or access-to-care initiatives.

  • Strong knowledge of coordinated access, centralized intake models, and surgical pathways within Ontario.

  • Experience coaching, mentoring, facilitation, and developing of staff to achieve best-practice standards.

  • Demonstrated ability to build and maintain collaborative relationships with internal and external stakeholders, including hospitals, primary care providers, community members, and surgical partners.

  • Excellent communication, presentation, and facilitation skills, with the ability to effectively represent the program at committees, working groups, and regional forums.

  • Ability to travel between sites and to meetings (2-5 times per month).


Additional Information

Hours: Monday to Friday, days,8-4, some evening hours (hours subject to change based on operational needs)


Salary: $54.219 to $67.80 per hour
