Job summary

CarePartners in Brampton, ON is seeking Team Coordinators to ensure effective customer service and communication between patients, families, and staff. The role oversees scheduling of PSWs, nurses, and therapists to meet service delivery standards, with full-time and part-time openings in Brampton.

Part-time roles include every-other-weekend shifts and opportunities to pick up additional shifts. The position offers incentives and perks, including paid orientation, health & dental benefits, EAP,

Qualifications

  • Post-secondary education preferred and at least 1 year of job-related experience (administrative or customer service).
  • Exceptional interpersonal and customer service skills.
  • Ability to work independently and in a team.
  • Strong proficiency in Microsoft Office (Word and Excel).
  • Non-traditional hours, including evenings and weekends, are required.
  • Experience in health care is an asset.
  • Clear background check.
  • Attention to detail and quick learner of new systems.

Responsibilities

  • Bridge between patients/caregivers, families, PSWs, supervisors, funders, finance and other CarePartners staff.
  • Receive routine referral/intake information from funders and private-pay clients.
  • Develop and coordinate service schedules with relevant factors.
  • Inform workers of new assignments and changes to schedules.
  • Enter information into the information system/database.
  • Produce schedules and reports on a regular basis and on supervisor request.
  • Handle inbound calls and email inquiries related to service.
  • Perform other administrative duties as required.

Overview

CarePartners is adding to our team and we are looking for Team Coordinators to ensure effective customer service and communication between patients, families, and staff. Our Team Coordinators ensure the scheduling of our PSWs, Nurses, and Therapists meets service delivery standards and administrative functions related to client services are performed in a timely manner.

We have immediate Full-Time and Part-Time openings in the Brampton, ON office!

Part-Time roles include shifts every other weekend and there is an opportunity to pick up additional shifts.

  • This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The hourly range is $19.67 to $26.40, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.
What The Role Involves
  • You’ll be the bridge between patients/caregivers, their families, Personal Support Workers, team supervisors, funders, finance and other CarePartners employees
  • Receiving routine referral/intake information from funders and private-pay clients
  • Developing and coordinates service schedules while considering relevant factors
  • Informing workers of new assignments and changes to their schedules
  • Entering relevant information into the automated information system/database
  • Producing schedules and reports on regular basis, and on request of the Supervisor
  • Receiving and manage inbound telephone calls and email inquiries related to service
  • Performing other administrative duties
  • Other duties as required
What You Bring
  • A strong desire and commitment to making a difference in the lives of their patients
  • Post-secondary education is preferred and one (1) year of job-related experience (such as administrative or customer service) is required
  • Exceptional interpersonal and customer service skills
  • Ability to work both independently and as part of a team
  • Strong proficiency in all aspects of Microsoft Office, with proven skills in Word and Excel
  • A strong attention to detail and the ability to learn new systems and databases
  • Effective verbal and written communication skills
  • Non-traditional hours, including evenings and weekends are required
  • Experience in the health care field an asset
  • A Clear background check
