Job summary

Ridge Meadows Hospital in Maple Ridge, BC, a Fraser Health Authority facility, is seeking a Patient Care Coordinator. You’ll supervise nursing staff, manage admissions and patient flow, and lead care-planning conferences to ensure efficient, safe patient care within a dynamic clinical team.

You will contribute to quality improvement, mentor staff, and collaborate across disciplines in a supportive, community-focused hospital setting under a new collective agreement offering flexible scheduling.

Qualifications

• Current practicing RN with BC College of Nurses & Midwives (BCCNM).
• 3 years recent clinical experience in designated area; 1 year supervisory experience.
• Post-graduate coursework or certification relevant to the role.

Responsibilities

• Manage admissions, intake prioritization and patient flow; plan discharge and appropriate placement.
• Supervise nursing and support staff; coordinate day-to-day operations and staffing levels.
• Promote interprofessional collaboration and team-based care; coach and mentor staff.
• Lead plan of care conferences/rounds (e.g., iCare) for optimal patient care management.
• Identify staff learning needs and contribute to development of learning objectives.
• Support quality improvement, risk management, and evidence-informed practice.
• Assist budget/resource planning and equipment needs; support optimal use of resources.
• Participate in audits, safety initiatives, and research/evaluation activities.
• Collaborate with internal/external stakeholders and committees; support policy implementation.
