CHATS-Community & Home Assistance to Seniors seeks a Home at Last Program Supervisor for a full-time, 35-hour work week. The role reports to the Integrated Care Manager, Hospital Programs and oversees the day-to-day operations, staffing, referrals, and partnerships to support safe hospital-to-home transitions.
Ideal candidates bring post-secondary education in health care or related field and at least 3 years of supervisory experience, with strong leadership and relationship skills.
Salary Range: $50,000.00 To $53,800.00 Annually
HOME AT LAST (HAL) PROGRAM SUPERVISOR
Full-time position
35 hours per week, Monday to Friday, 8:30am to 4:30pm
Mid August start
The successful candidate must be available to work occasional evenings and weekends to support program operations.

Reporting to the Integrated Care Manager, Hospital Programs, the Home at Last Supervisor oversees the day-to-day operations of the Home at Last program, ensuring the delivery of high-quality, client-centered services that support safe and timely transitions from hospital to home. The Supervisor provides leadership and support to program staff; manages referrals, scheduling, staffing resources, and program capacity; and builds strong collaborative relationships with hospitals, Ontario Health at Home, and community partners to facilitate seamless care transitions.
