Job summary

University Health Network (UHN) invites applications for a Part-Time Social Worker in the Spinal Cord Rehab Program (Outpatient Services) at Toronto Rehab Institute - Lyndhurst Centre. This role involves intake, therapy, and collaboration within an interdisciplinary team to support patients and families.

The ideal candidate holds a Master’s in Social Work, current Ontario registration, and experience in rehabilitation and counselling.

Qualifications

• Master’s degree in Social Work from an accredited university.
• Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers.
• Certification in CBT, MI, and MBSR is preferred.
• Membership in the Ontario Association of Social Workers (OASW) is preferred.
• Experience in rehabilitation with neurological populations is required.
• Experience with individuals with Spinal Cord Injury is preferred.
• 2+ years experience in outpatient hospital settings preferred.
• Experience in group and individual counselling is required.

Responsibilities

• Participate in intake and transitional care processes.
• Provide individual and group therapies to clients and families, including psycho-education, relaxation, stress management, problem solving, sleep hygiene, and self-advocacy.
• Communicate pertinent information and recommendations to clients/care providers and provide education.
• Participate in, or support a virtual model of therapeutic care as required.
• Maintain up-to-date charting/records in compliance with policies and standards.
• Participate in developing and implementing new therapeutic programs based on clinical needs.
• Participate in research, program development and evaluation, and quality assurance activities.
• Collaborate with interdisciplinary health team to plan and evaluate patient care.

Education

Master’s Degree in Social Work

Certificate of Registration with the Ontario College of Social Workers and Social Service Workers

CBT, MI, MBSR certifications preferred

OASW membership preferred
