Job summary

CBI Home Health invites a Speech Language Pathologist to join our Integrated Care Program in the Peterborough region, delivering at-home assessments and interventions to support patients in the community. This role emphasizes collaboration with an interdisciplinary team and a person-centered approach to care.

The position offers flexible scheduling, travel support, and opportunities for professional growth within Canada’s national CBI Health network.

Qualifications

• Must be registered in good standing with the College of Audiologists and Speech Language Pathologists of Ontario.
• Valid driver’s license and vehicle insurance.
• Ability to deliver care in person and via virtual methods.

Responsibilities

• Assess and implement SLP interventions in home-based settings (swallowing, dysphagia, communication).
• Collaborate with interdisciplinary care teams to plan and deliver services.
• Support holistic, restorative care including stroke pathways and cognitive communication.
• Utilize digital health platforms to support care delivery, documentation and intervention.

Salary Range: The salary range for this position is $117.10 - $132.30 per visit

What You Will Do

• Assessment and implementation of SLP interventions (i.e. swallowing assessments, dysphagia management, communication, etc.) in a home-based setting
• Use of a collaborative and interdisciplinary team-based approach to care and services
• Support a holistic and restorative approach to care (i.e. enhanced stroke pathway; cognitive communication supports, etc.)
• Leverage our digital health platforms to support care delivery, collaboration, documentation, and intervention

What You Will Need To Be Successful

• You are registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
• Valid driver's license and vehicle insurance
• Your focus is to embrace person and family-centered care
• You are comfortable delivering care both in person and using virtual methods/technology
• You are passionate, empathetic, use good judgement and apply critical thinking when making client decisionsYou are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
• You are flexible and have demonstrated skills in critical thinking that you can apply to a variety of situations.
• You value the role a therapist plays in keeping clients at home, where they want to be
