Job summary

University Health Network (UHN) – Toronto Rehab Institute is seeking a part-time Social Worker for the Spinal Cord Rehab Program. You will participate in intake, provide individual and group therapies, and contribute to care planning with an interdisciplinary team.

The role requires a Master’s in Social Work, active registration in Ontario, and experience with neurological rehab. 15 hours/week; competitive compensation; temporary 1-year contract.

Qualifications

  • Master’s degree in Social Work from an accredited university.
  • Current Certificate of Registration in good-standing with the Ontario College of Social Workers and Social Service Workers.
  • Experience in rehabilitation with neurological populations.
  • Experience with spinal cord injury is preferred.
  • Minimum 2+ years in outpatient/hospital settings.
  • Experience in group and individual counselling is required.
  • Excellent computer skills with Microsoft Office and patient information systems.
  • Strong interpersonal and communication skills; leadership and problem-solving abilities.

Salary:$43.47 - $54.34 Hourly (To be commensurate with experience, and consistent with UHN compensation policy)
Status:Temporary Part-Time (1 year)

Duties

  • Participate in intake and/or transitonal care processes.
  • Provide individual and group therapies to clients and families, including the provision of support with respect to acceptance and loss, psycho‑education, and skill building in areas such as relaxation, stress management, problem solving, sleep hygiene, and self‑advocacy.
  • Communicate pertinent information and recommendations to clients/care providers and provide education.
  • Participate in, or support a virtual model of therapeutic care as required.
  • Maintain up-to-date charting/records in compliance with UHN, departmental policies and procedures, and the Ontario College of Social Workers and Social Service Workers standards of practice.
  • Participate in developing, planning, and implementing new therapeutic programs based on clinical needs.
  • Participate in research, program development and evaluation, and quality assurance activities.
  • Actively pursue learning opportunities and communicate new developments and best practices through education sessions with team members.
  • Develop an intervention plan in collaboration with patients, families, stakeholders, and the interdisciplinary team, based on identified needs.
  • Document intervention plans according to specific UHNand professional standards.
  • Assist in overcoming barriers within the health care system to ensure timely delivery of services.
  • Help patients identify and use health resources, including those available in the community and through agencies.
  • Liaise with community agencies and assist with making connections to community resources/supports as required.
  • Provide short-term counselling services, with particular regard to adjustmentissues.
  • Balance group, same day, urgent, and individual counselling.
  • Collaborate with members of an interdisciplinary health team to plan, implement, coordinate, and evaluate patient care in consultation with patients and families.
  • Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  • Educate and promote health, safety and wellness in the work place.
  • Represent the department or program on various committees and in meetings as required.
  • Perform other duties as required.
  • Minimum of a Master’s Degree in Social Work from an accredited university.
  • Current Certificate of Registration in good-standing with the Ontario College of Social Workers and Social Service Workers.
  • Certification in Cognitive Behavioural Therapy (CBT), Motivational Interviewing (MI), and Mindfulness‑based stress reduction (MBSR) is preferred.
  • Membership in the Ontario Association of Social Workers (OASW) ispreferred.
  • Experience in rehabilitation with neurological populations is required.
  • Experience with individuals with Spinal Cord Injury ispreferred.
  • 2+ years experience in an outpatient, hospital-setting preferred.
  • Experience in group and individual counselling is required.
  • Demonstrated leadership and problem‑solving skills.
  • Demonstrated ability to liaise with other health care professionals, programs, and external agencies.
  • Demonstrated ability to mentor students and support personnel.
  • Demonstrated knowledge of Medical Terminology.
  • Demonstrated excellence injudgment and proven analytical skills.
  • Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  • Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  • Demonstrated superior interpersonal and communication skills, both written and verbal.
  • Demonstrated effective time management and organizational skills with the ability to organise and prioritise as required.
  • Demonstrated discretion and maturity when handling confidential information.
  • Demonstrated commitment to the safety of co‑workers and patients.
