Job summary

AgeCare in Alberta is seeking a Social Worker to support residents and families, guiding move-in processes, transitions, finances, health concerns and access to community resources. You will coordinate with families and POA when needed and contribute to a resident-centered environment.

You will assist the Director of Care and Business Manager, participate in CQI activities, and prepare reports for the residence and corporate office as required.

Qualifications

• Bachelor's or Master's Degree in social work.
• Minimum one-year relevant experience preferably in a health care setting.
• Previous experience in a long-term care setting is preferred.

Responsibilities

• Assist residents and families with move-in processes, transitions, finances, concerns and health declines.
• Provide access to community resources and support as needed.
• Participate in the residence’s Continuous Quality Improvement (CQI) program and prepare/reports for residence and corporate office.
