Job summary

Liquid Personnel is recruiting a Social Worker for its Older People’s Team in Surrey. The role focuses on delivering strengths-based interventions to promote independence and wellbeing for older adults and their families.

You will undertake Care Act assessments, reviews and care planning, manage caseloads, and collaborate with multidisciplinary teams across health and social care to safeguard and support service users.

Qualifications

• Registered with Social Work England.
• Eligible to work in the UK.
• Hold a full UK driving licence.
• Post-qualified experience and recognised social work qualification.

Responsibilities

• Undertake Care Act 2014 assessments, reviews and reassessments.
• Complete strengths-based, person-centred assessments and support planning.
• Manage a caseload of adults with learning disabilities and autism.
• Coordinate care and support services to meet assessed needs and promote independence.
• Work collaboratively with multidisciplinary teams, health partners, carers and families.
• Advocate for service users' rights, choices and wellbeing.
• Undertake Mental Capacity Assessments and contribute to Best Interest decisions.
• Identify and respond to safeguarding concerns in line with statutory procedures.
• Assess and manage risk while promoting positive outcomes and independence.
• Maintain accurate, timely and legally compliant case records and reports.

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
