CBI Home Health in Hamilton is hiring Social Workers to join an integrated care model, delivering hospital-level support to home-based patients. You’ll collaborate with hospital and community teams to help clients recover, regain independence, and improve outcomes in their own homes.

The role requires a Bachelor or Master of Social Work, active registration with the Ontario College of Social Workers and Social Service Workers, and travel within the Hawkesbury area as part of the care team.

Qualifications

• Bachelor of Social Work or Master of Social Work is required.
• Registered with the Ontario College of Social Workers and Social Service Workers in good standing.
• 1–2 years of community experience is an asset.
• Must be passionate, empathetic, and able to apply good judgement and critical thinking.
• Ability to travel within Hawkesbury community as part of the Home Health Services team.

Salary Range: The per visit rate range for this position is $77.68-$86.32

What you will need to be successful

• You have a Bachelor of Social Work or a Master of Social Work
• You are a registered member in good standing with the Ontario College of Social Workers and Social Service Workers
• 1 to 2 years of community experience is an asset
• You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions
• You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
• You have ability to travel within the Hawkesbury community as part of the CBI Health Home Health Services team
