Job summary

Autism Services in Saskatoon is seeking a Pay Band Social Worker - BSW to join the Connecting to Care team on a temporary full-time basis.

The Case Manager provides social case management support as a primary care provider to clients who face engagement barriers related to substance use and mental health. A BSW and at least 1 year in health settings are required, with a valid Class 5 license and the ability to work days/evenings/weekends.

Qualifications

• Bachelor of Social Work (BSW) is required.
• Minimum 1 year experience in a health-related field.
• Valid Class 5 driver's license.

Job Description: The Case Manager provides social case management support as a primary care provider to the client. The population for this project are those who have difficulty engaging or have disengaged from care, involved in substance abuse/who are impacted by substance use disorder and Mental Health issues. Utilizing an outreach approach, the case manager provides client services including psychosocial assessments on intake, addiction assessments, client goal setting/skill building, and coordination/support of health/social services. A model of recovery services is utilized for those challenged with an addiction. The case manager acts as a client advocate while working with other health care professionals to provide services. The goal is to help clients establish a community based medical team and participate in collaborative planning, to achieve the best health possible, that may include stabilizing their mental Health and substance use disorders. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"
