Job summary

Centre for Complex Diabetes Care (CCDC) in Ontario is seeking a Social Worker to provide advocacy, assessment and psychosocial support to clients managing diabetes.

You will collaborate with an interdisciplinary team, develop individualized care plans, assist with social determinants of health, and connect clients to healthcare and community resources. Some travel to remote communities may be required.

Qualifications

• Post-secondary education in Social Service Work, Social Work, or equivalent required.
• Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), or eligible to obtain required.
• Canadian Diabetes Educator Certification (CDE/CDCES), or eligible and willing to obtain certification.
• Knowledge of community resources and experience working with community organizations.
• Excellent interpersonal, written, and verbal communication skills.
• Ability to work independently and as part of an interdisciplinary team.
• Strong assessment, problem-solving, and case management skills.
• Knowledge and sensitivity to First Nations culture and communities is an asset.
• Training or experience in motivational interviewing, crisis intervention, and/or cognitive behavioural therapy (CBT) is an asset.
• Experience in a healthcare setting is an asset.

Responsibilities

• Provide case management and psychosocial support services to adults and children living with diabetes.
• Assess patient needs and develop individualized care plans in collaboration with patients, families, and the interdisciplinary team.
• Deliver supportive counselling, crisis intervention, advocacy, and referrals to appropriate community resources and services.
• Assist patients in addressing social determinants of health, including accessing financial, social, and community supports.
• Act as a navigator to connect patients and families with healthcare, social assistance, and community services.
• Work collaboratively with members of the diabetes team and broader circle of care to support optimal patient outcomes.
• Maintain current knowledge of community resources and diabetes-related supports.
• Document patient care activities accurately and maintain required statistics and reports.
• Promote healthy coping strategies, self-management, and improved functioning in the community.
• Maintain effective communication with patients, families, healthcare providers, and community partners.
• Travel to remote and isolated communities by vehicle and aircraft, including occasional overnight travel, may be required.

Job description

The Centre for Complex Diabetes Care (CCDC) consists of an interdisciplinary team who work together to empower and support clients living with diabetes while maintaining safe, holistic, and patient-centered care. The Social Worker provides advocacy, assessment and support to clients who are managing the psychosocial impacts of living with diabetes. The Social Worker will practice within the standards set by the College of Social Workers and Social Service Workers (OCSWSSW).

Salary :

Starting at $69,225 – $83,343. Commensurate with skills, experience and education.
