SLMHC in Sioux Lookout, Ontario is seeking a CCDC Social Worker to provide assessment and intervention for psychosocial needs related to diabetes, working with clients and families through individual, family, and group modalities.
The role involves collaborating with an interdisciplinary team to support diabetes self-management in a holistic, patient-centered manner and to promote optimal outcomes in the community.
Following the scope of practice as set out by the College of Social Workers and Social Service Workers (OCSWSSW), the CCDC SW is responsible for providing assessment and intervention to address the psychosocial needs of the clients and families through individual, family and/or group modalities. As part of CCDC, the SW will work collaboratively with other professionals and all members of the interdisciplinary care team to support and enhance diabetes self-management for the client while maintaining safe, holistic, and patient centered care.
Starting at $69,225 – $83,343. Commensurate with skills, experience and education.
