Job summary

Windsor Essex Community Health Centre is seeking a Registered Dietitian for the Diabetes Wellness site (2885 Lauzon Parkway). The role focuses on diabetes nutrition assessment, plan development, education, and collaborative care across the inter-professional team.

Qualifications

CORE RESPONSIBILITIES OF THE POSITION

The Registered Dietitian functions as a member of the inter-professional team and performs nutritional assessments, development of client-focused diabetes nutritional plans, health education/counseling, and other clinical activities according to the College of Dietitians of Ontario standards of practice and Diabetes Canada guidelines. The Registered Dietitian will also:

• Assess client nutritional status by gaining an understanding of the client’s goals, psychological outlook, cultural background and clinical profile.
• Provide written and verbal instructions to clients, translating scientific information about healthy diabetes eating into practical dietary advice for people based on their goals and lifestyles.
• Educate and advise a wide range of clients with pre-diabetes, Type 1 and Type 2 Diabetes based on the client’s goals and practical ways in which they can improve their health by adopting healthier habits and facilitating self-management.
• Participate as a member of the inter-professional team in all functions such as planning and team meetings.
• Contribute to chart documentation as per College of Registered Dietitians of Ontario Standards, weCHC policies and practices; participate in chart reviews and case conferences to referring/non-referring Physicians, Endocrinologists or primary health care provider.
• Perform assessments and develop diabetes nutritional plans incorporating clients’ health goals, culture and income.
• Network with internal and community partners to ensure an optimal continuum of health and social services for clients/patients.
• Review client referral from prescribing physician/endocrinologist, as well as the client’s eligibility for treatment/nutritional counselling and consult with the physician if the client is found ineligible for treatment and provide rationale.
• Work collaboratively with prescribing primary care providers, endocrinologists, and other health care providers to provide optimal client care.
• Public speaking in various venues as required.
• Involve client and family in all aspects of their nutritional care and lifestyle by monitoring client progress on a regular basis and working within the client’s circle of care to ensure positive client outcomes.
• Communicate client progress to referring/non-referring Physicians, Endocrinologists, or Primary Healthcare providers as needed.
• Perform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.
• Participate in quality improvement of the program and organization, as required.

SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED

• 3-5 years clinical counselling and community diabetes nutrition experience required.
• Strong knowledge of nutritional issues affecting clients, thorough knowledge of community-based health care and resources.
• Experience working with clients with an emphasis on diabetes and other chronic diseases.
• Must have or be working towards a Certified Diabetes Educator (CDE) designation.
• Strong knowledge of diabetes affecting adults with Type 1, Type 2, gestational and prediabetes clients.
• Excellent communication and presentation skills with a demonstrated ability to undertake diabetes nutritional/general counselling (individual and group).
• Demonstrated ability to work independently and as a team member within an evolving role in an inter-professional environment.
• Ability to work in a variety of social and physical settings during regular business hours and in the evening. Occasional weekend work may be required.
• Experience in program development, implementation, monitoring and evaluation.
• Excellent follow-up and follow through skills with ability to identify and resolve problems in a timely manner.
• Proficiency in the use of computers/various software applications, including, Microsoft Office programs.
• Completion of a Public Health Unit, Food Handler Course, or equivalent program is an asset.
• Bilingual (English/French/Arabic) language skills an asset.
• Current, valid Class "G" Driver's License and reliable transportation to support travel across Windsor-Essex County, as required

Education and certificate requirements

• Bachelor of Science in Nutrition and Food from a recognized university
• Member in good standing and current registration with the College of Dietitians of Ontario
• Working towards or completion of Certified Diabetes Educator (CDE) designation
• Completion of a police clearance and vulnerable sector check prior to commencing employment
