Job summary

Cornwall Community Hospital is seeking a Social Worker BSW to join the Social Work unit in Cornwall. You will provide clinical social work services, perform assessments, discharge planning, and coordinate community referrals within a hospital setting.

The position is permanent full-time with one opening. Eligibility for registration as a Social Worker in Ontario is required; OPSEU represents the union. The salary range is shown as hourly CAD in the posting.

Qualifications

• BSW degree and eligibility for social worker registration.
• Experience in hospital or clinical social work is desirable.
• Strong communication and discharge planning skills.

Responsibilities

• Provide clinical social work services to inpatients and families.
• Conduct psychosocial assessments and care coordination.
• Plan and facilitate discharge and community referrals.
