A leading health authority is seeking a dedicated Social Worker to provide psychosocial care to residents. The role involves assessing needs, collaborating with healthcare teams, and advocating for residents' well-being. Ideal candidates will have a Bachelor's in Social Work and experience in healthcare settings, particularly with gerontology or chronic diseases.

Reporting to the Clinical Coordinator or Social Work Manager, the Social Worker assesses and provides services to meet the emotional and social needs of an assigned group of residents and their families. Collaborating with other healthcare providers and the multidisciplinary team, the Social Worker offers direct psychosocial care, system navigation, and advocacy. The role involves developing social work services to maintain or enhance the health, social well-being, and functioning of residents.
Physical ability to perform the duties of the position is required.
