Job summary

University Health Network is seeking a Personal Support Worker (PSW) for the Toronto Western Hospital site. The role involves assisting patients with activities of daily living, lifts and transfers, and documenting care under the supervision of nursing staff.

Candidates should have HSCPOA registration and a current BCLS certificate; acute care or rehab experience is an asset. The position offers flexible shifts and casual status with non-negotialble start rate details in the posting.

Qualifications

• Current registration with HSCPOA is required.
• Current Basic Cardiac Life Support (BCLS) certificate required.
• Experience in acute care and/or rehab is an asset.

Responsibilities

• Assist patients with activities of daily living (feeding, lifts and transfers, bathing, skin care, oral hygiene, toileting).
• Perform tasks delegated by an RN or RPN in accordance with RHPA with proper training.
• Communicate basic information according to the Plan of Care.
• Maintain a safe environment by reporting unsafe conditions and changes in patient status.
• Complete and maintain patient records and progress notes as per policy.

Position Summary

The Personal Support Worker (PSW) is a member of the inter-professional care team and is expected tomaintaina collaborative and active working relationship with team members in the provision of patient care.

Duties

• Assistpatients with activities of daily living: This includes feeding, lifts and transfers, bathing, skin care, oral hygiene, and toileting.
• Perform tasks assigned or as delegated by an RN or RPN in compliance with the RHPA for whichappropriate educationand training has been completed. RNs and RPNs willbe responsible forthe oversight of the PSW when performing the assigned or delegated tasks. Delegated tasks can include actions that involve insertion of a digit or instrument into a body cavity, or any care or procedure under the dermis, for example.
• Communicate anddemonstratebasic informationin accordance witha patient’s pre-established Plan of Care.
• Ensure a safe environment for patients and staff by reporting and documenting unsafe conditions such as broken equipment as well as any patient behavioral, physical, or cognitive changes. Safety concerns must be reported to an RN, RPN or other regulated health provider, in compliance with patient safety responsibilities and corresponding hospital plans, policies and proceduresin order toensure a safe environment for patients.
• Complete andmaintainrelated patient records and progress notes, as per hospital policy.

• Current registration with the Health and Supportive Care Providers Oversight Authority (HSCPOA);
• Current Basic Cardiac Life Support (BCLS)Certificate;
• Behavioral management training (such as PIECES, U-First, CPI) is considered anasset
• Ability to handle daily mild to moderate physical exertion (i.e.constant standing, walking, climbing, reaching and frequent movement) on an occasional to frequent basis;
• Acute care and/orpreviousrehab experience considered anasset;
• Excellent computer skills;
• Good attendance record isrequired
