We are seeking a Temporary Full-Time Social Worker to join our team at the Wilson Site, reporting to the Manager, Access and Flow. The role is scheduled until December 31, 2025, with the possibility of extension, requiring availability on weekdays, weekends, and statutory holidays on a rotational basis. The hourly rate ranges from $44.67 to $57.90.

Position Responsibilities
  • Provide psychosocial assessment, crisis intervention, discharge planning, and counseling to a diverse patient population, including inpatient acute medical, ICU, surgical, emergency, and geriatric patients.
  • Act as liaison and assist patients and staff with community resources.
  • Participate in discharge planning and patient flow management within acute and community sectors.
  • Work collaboratively with the healthcare team and supervise social work students.
  • Provide cross-coverage of social work services as needed, including at Reactivation Care Centres.
Qualifications
  • MSW from an accredited school of Social Work.
  • Current membership or eligibility for membership with OASW and OCCSWSSW.
  • Experience in acute/sub-acute settings, patient flow, and discharge planning is preferred.
  • Proficiency in computer skills and electronic record management.
  • Knowledge of healthcare sectors and community resources.
  • Strong organizational, interpersonal, and communication skills.
  • Ability to work independently and in a team.
  • Cultural sensitivity and excellent attendance record.
