Job summary

VHA Home HealthCare in Ontario is seeking a Social Worker to provide assessment, counseling and planned interventions across diverse client groups in home and community settings.

You will assess psychosocial needs, deliver short-term therapy, crisis intervention, and coordinate care with the multidisciplinary team, documenting outcomes and discharge plans. We value inclusivity, ongoing professional development and accessible care for all clients.

Qualifications

  • Master of Social Work (MSW) degree and OSWSSW registration.
  • At least two years of clinical social work experience.
  • Specialized courses to support clinical expertise.
  • Knowledge of community-care requirements and available resources.
  • Experience with using outcome measures to support interventions.

Responsibilities

  • Assess clients and families to identify factors affecting care and coping.
  • Counsel clients and families on long-term care planning and in-home options.
  • Provide short-term therapy and crisis intervention as needed.
  • Coordinate care across the client’s team and document plans and progress.
  • Maintain accurate assessment findings, treatment plans, and discharge summaries.

I feel welcome and at home.”View more comments from our clients and their family members.Pay RangeVHA's Social Worker provides consultation, assessment and planned interventions for diverse clients groups across Southern Ontario and the Capital region, referred through VHA Rehab Solutions and Champlain Rehab Solutions.
