Job summary

VHA Home HealthCare in Ontario is a not-for-profit home care provider serving seniors, families and people with disabilities to live independently at home through in-home care and community services.   • Assist with walking, transfers, stairs, safe mobility, and positioning using proper body mechanics.
  • Provide client/family education and support to address physical, functional and self-care needs as assigned.
  • Monitor and report changes in health status to the Physiotherapist and/or Occupational Therapist.
  • Document in the client record and collect required data as requested.
  • Adheres to policies and procedures related to safe client care.
  • Participate in program planning, quality improvement and evaluation.

Responsibilities
  • Pay Range Assist and supervise the client with his/her individual prescribed exercise programs or ADL training.
  • Assist and supervise client with walking, transfers, stairs, safe mobility, and positioning using proper body mechanics.
  • Provide client/family education and support to address physical, functional and self‑care needs as assigned.
  • Assist with gross motor, fine motor, and balance activities.
  • Monitor and report changes in health status to the Physiotherapist and/or Occupational Therapist.
  • Participate in program planning, quality improvement and evaluation.
  • Document in the client record and collect required data as requested.
  • Supports safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
  • Adheres to policies and procedures related to safe client care.
  • Applies core client safety knowledge, skills, and values to everyday work.
  • Adheres to applicable professional practice standards.
  • As relevant, evaluate clients’ environmental and personal safety risks through in‑home safety assessments/falls risk assessments and other assessments as required.
  • Assumes responsibility for own health and safety by adhering to required policies and best practices.
  • Ensures that risk events, critical incidents and accidents are reported in accordance with VHA policy and Ontario law.
  • Adheres to organizational policies and procedures regarding workplace harassment and violence.
  • Reports all incidents in which a person is threatened, abused, or assaulted in circumstances related to their work, including all forms of harassment, violence, bullying, intimidation, physical threats or assaults, robbery or other intrusive behaviours at any level of the organization including criminal intent, client or customer relations, worker‑to‑worker or personal relationships.
  • Ensures proper use of personal protective equipment (PPE) and clothing required by VHA.
  • Employs all best practice infection prevention and control measures in accordance with VHA policy and procedures.
