Social Worker for a 155-bed Long-Term Care Home
Part-time contract position, 15 hours weekly. Flexible work schedule.
Must Haves:
- Graduation from a Bachelor of Social Work program
- Current registration with the BC Association of Social Workers
Duties & Responsibilities:
- Coordinate Move-In/Move-Out of Residents in collaboration with clinical leadership, including evaluation and assessment of individual needs and appropriateness of placement.
- Responsible for Pathways System and communication with health services’ Transition Services.
- Provide transitional support to family and Residents upon Move-In, documenting as per policies.
- Participate in Move-Out planning and follow-up as necessary.
- Promote Resident autonomy in decision-making regarding health care, personal, and financial matters.
- Assist Residents and families in sourcing resources related to decision-making, guardianship, and legal directives, in collaboration with the Site Leader.
- Help Residents and families utilize community resources to enhance quality of life.
- Contribute to developing and evaluating Resident care plans as part of an interdisciplinary team, advocating for Residents and families.
- Participate in care conferences and assist in resolving family issues and complaints.
- Complete required documentation promptly, including social history, assessments, and progress notes.
- Attend and participate in committees and meetings such as Leadership Team and End of Life Committee.
- Engage in in-services, orientation, and employee training as requested.
- Lead quality improvement and performance measurement initiatives, including follow-up and action plan implementation.
- Collaborate with the Recreation Department to plan and conduct Memorial Services.
- Perform other duties as required.
- Coordinate and participate in MDS data entry, communicating with staff, health professionals, and families.
- Manage MDS and Resident Assessment Protocols related to Move-Ins, Move-Outs, transfers, and within scope of practice.
Candidate must be a team player and able to work independently. Experience in residential care is an asset but not required.
