Job summary

Stella's Circle is seeking a seasoned social worker in Canada to provide psychosocial assessments, plan and monitor community services, and advocate for clients within a fast-paced setting. You will collaborate with an interdisciplinary team to ensure client-centered care and ethical practice.

The role involves client interviews, program evaluation, and referrals to housing, legal aid, medical treatment, and other supports, with emphasis on outcomes and accountability.

Qualifications

  • Bachelor's degree in social work or related field.
  • Minimum 5 years of relevant experience.
  • On-site work at location; no remote option.
  • Membership in a provincial association of social workers.
  • Criminal record check, enhanced reliability security clearance, vulnerable sector check.
  • Adults are the target audience.

Responsibilities

  • Conduct psychosocial assessments.
  • Evaluate program effectiveness.
  • Interview clients individually, in families, or in groups, to assess their situation and needs.
  • Prepare detailed plans for clients returning home after rehab.
  • Provide mediation services.
  • Perform administrative tasks.
  • Provide counselling and therapy to help clients resolve social and personal problems.
  • Plan programs of assistance for clients.
  • Refer clients to housing, legal aid, medical treatment and other services.
  • Serve as a member of an interdisciplinary team of professionals.
  • Advocate for clients in courts or hearings.
  • Conduct social research.
  • Evaluate counselling outcomes.
  • Develop prevention and intervention programs to meet community needs.
  • Monitor and evaluate programs.
