The Social Worker counsels and provides guidance to individuals and/or families with social, medical, and/or psychiatric challenges, under the guidance of the Director of Care.

Responsibilities

1. Interview residents to determine the nature and degree of problems, disabilities, or illnesses and interpret data.
2. Assist family members in understanding residents’ needs and help residents and families work out realistic functional goals.
3. Support residents and families through individual or group conferences to understand, accept, and follow medical recommendations.
4. Assist residents and families with social functioning issues affected by disability.
5. Collaborate with multidisciplinary teams in diagnosis and treatment plans.
6. Conduct psychosocial screening activities to determine the need for specialized psychiatric assistance.
7. Serve on committees as required.
8. Perform other related duties as assigned.

Qualifications and Experience

• Must be a Registered Social Worker or eligible for registration.
• Bachelor’s degree in social work, Counseling, Psychology, Behavioral Science, or related field.
• Minimum 2 years experience in social services or related capacity, or an equivalent combination of education and experience.
• Knowledge of social work principles, practices, techniques, and standards.
• Understanding of community organizations and social service programs.
• Strong counseling and conflict resolution skills.
• Exceptional interpersonal skills with peers, residents, visitors, and operational partners.
• Solid organizational skills, with the ability to prioritize and problem-solve.
• Proficient in MS Office Suite and record-keeping software.
• CPR and First Aid certification or willingness to obtain.
