Job summary

Casey House in Toronto is seeking an Inpatient Social Service Worker to join the inpatient team on a full-time, temporary basis. You will work within an interprofessional team to coordinate care, provide information and system navigation for clients and their networks.

Qualifications

• Diploma in Social Service Work or an undergraduate degree in Social Work.

• Experience with populations living with HIV or at risk, and understanding of social determinants of health.
• Minimum 3 years of experience with clients who have complex medical, mental health, and substance use needs.
• Experience working with diverse and marginalized populations (racialized, Indigenous, 2SLGBTQIA+).
• Experience providing case management and advocacy in community settings; client empowerment focus.
• Excellent communication, advocacy and interpersonal skills.

Responsibilities

• Conducts assessments to develop individualized client care plans.

We currently have an opportunity for one (1) Social Service Worker to join our Inpatient team in Toronto, ON, on a full-time, temporary basis (OPSEU bargaining unit)

Reports to:Inpatient Clinical Manager

Hours:Primarily Monday to Friday, 37.5 hours per week, with weekend and/or evening work as scheduled. This is a fully on-site position. All scheduled shifts take place on the Inpatient Unit.

Compensation:$34.34 to $42.78 per hour (7-step grid)

Position Summary:

The Inpatient Social Service Worker is an integral member of an interprofessional team, responsible for providing information, education, and system access and navigation to inpatient clients and their networks of support. This role collaborates closely with the inpatient social worker to address client needs and support continuity of care. The SSW works directly with clients to build essential life skills and promote independence, enhance quality of life, and strengthen their ability to access and engage with community resources. This role emphasizes a strengths-based, client-centred approach that recognizes and builds upon individuals' resilience, capabilities, and existing support.

Responsibilities

Case Management, Client Support, and Advocacy

• Conducts assessments to develop individualized client care plans.
• Initiates referrals to community-based services matched to client needs.
• Responsible for organizing discharge planning meetings and coordinating related activities, ensuring the involvement of the inter-professional team and appropriate community resources including our outpatient program.
• Collaborates closely with the interdisciplinary team, including peers, to provide comprehensive support for clients during and after discharge.
• Identifies SSW and client directed goals and documents them to plan of care.
• Initiates processes related to the client obtaining identification, health card, and any documentation to support their access to health and community-based services.
• Documents informed consent provided by the client for the team to engage with their community providers, informal supports, and significant others.
• Ensure compliance with Casey House policies, procedures, ethical guidelines, and professional standards.
• Provides coaching or educational sessions to help clients develop life skills like budgeting, time management, communication, and decision-making to support their independence and well-being.
• Provides harm reduction education and support including safety planning around substance use.

Interdisciplinary Collabration

• Participates in weekly interdisciplinary team meetings for the inpatient program, as well as weekly social work meetings and care plan rounds across departments, to enhance communication and collaboration between teams.
• Networks with community organizations to develop meaningful partnerships to meet the needs of the Casey House client population.
• Maintains professional development through seminars, workshops, and professional affiliations as approved by the Inpatient Clinical Manager.

Qualifications

Education and Experience

• Diploma in Social Service Work or an undergraduate degree in Social Work.
• Current registration in good standing with the Ontario College of Social Workers and Social Service Workers.
• Experience working with populations living with or at risk of HIV, along with an understanding of the social determinants of health that affect these communities, such as houselessness, food insecurity, and unemployment.
• A minimum of 3 years of experience working with clients who have complex medical conditions, mental health challenges, and substance use concerns, utilizing a strengths-based approach.
• Experience working diverse and historically marginalized populations, including racialized, Indigenous, or 2SLGBTQIA+_communities
• Experience providing case management and advocacy in community-based settings, prioritizing client empowerment.
• Excellent communication, advocacy and interpersonal skills

Technical Skills

• Proficiency in using EMR systems and case documentation tools. Knowledge of EPIC EMR is an asset.
• Knowledge of community resources, harm reduction principles, and trauma-informed care.
• Strong assessment, service planning, and client advocacy skills.
