Position: Social Worker, Full-Time, Day Shift (9am-5pm)
Home: Regency Long-Term Care
Location: Port Hope
Key Responsibilities
Receive and respond appropriately to inquiries pertaining to Southbridge Roseview which relate to new resident placement, including conducting tours of the home with prospective residents;
Conduct pre-admission procedures including: Screening of applications; Correspondence with applicants to acknowledge receipt of application and to provide information; Pre-admission visits and assessments; Communication with placement agencies; Navigate the resident/family through the admission process including coordination with the LHIN;
Promote and facilitate a Resident-oriented environment, acting as a Resident/family advocate. Coordinate and serve as the home's liaison person for Family Council. Lead the Customer Service program within the home;
Coordinate, plan and participate in annual and new admission interdisciplinary team care conferences for residents;
Plan, organize and carry out social work programs/services to meet the ongoing psychosocial needs of the Residents and their families/advocates;
Provide palliative care support to residents and family as needed;
Examine and act on matters concerning possible financial abuse or emotional abuse of a resident and work collaboratively with the interdisciplinary team to address the issue;
Lead the ethics committee for the home and be available to staff to consult regarding ethical dilemmas within the home, working collaboratively with staff to resolve these dilemmas;
Participate on committees related to resident care and operation of the facility as required;
Function as a member of the multi-disciplinary team in planning overall Resident care;
Provide educational and consultative services on social work related issues/topics through participation in staff orientation programs, staff development programs and educational seminars for staff, Residents and families/advocates as required.
Key Qualifications
Registered under the Social Work & Social Service Act, 1998; Graduate of a recognized school of social work with a Bachelor of Social Work degree;
Experience in working with Geriatrics; Familiarity with the multi-disciplinary team concept, skill in teamwork and ability to work effectively in a multi-disciplinary setting;
Minimum of 3 years' experience as a Social Worker in a clinical setting;
Strong knowledge base for mental health issues;
Excellent team player with strong interpersonal and communication skills;
Ability to respond to crisis situations with demonstrated critical thinking;
Demonstrated ability to function as a team player;
Good communication and interpersonal skills;
French Language skills will be considered an asset.
