Job summary

Qualifications

• Graduation from a recognized/approved Bachelor's Degree in Social Sciences/Humanities or Business is required.
• Knowledge of financial assessments for home supports, medical discharge, long-term care, assistance and/or subsidy programs.
• Knowledge of community support agencies, programs and eligibility requirements.
• Knowledge of complex health concerns.
• Demonstrated strong critical thinking, problem solving and time management skills.
• Demonstrated strong advocacy skills.
• Demonstrated ability to work independently.
• Demonstrated strong interpersonal skills with clients/patients/families and various community resources.
• Demonstrated strong oral and written communication skills.
• A condition of employment includes the use of personal vehicle and a Class 5 driver’s license.
• A satisfactory record of work performance is required.

Job Summary

This position reports to the Division Manager and the primary responsibilities of this position are to provide resource counseling and social support services to clients within the Personal Care Home Program.
