Job summary

Extendicare is seeking a RAI-MDS Coordinator for Stoneridge Manor in Carleton Place, Ontario. You will oversee the assessment process with the interdisciplinary team to ensure accurate, timely assessments and translate results into individualized care plans and quality improvement.

You will coach staff on RAI‑MDS, InterRAI LTCF, PointClickCare and Point of Care, while ensuring CIHI submissions and ongoing staff education.

Qualifications

• Current registration or licensure as a regulated health care practitioner.
• At least three years in clinical practice, project leadership or management.
• Experience with RAI‑MDS and interest in resident assessment.

Responsibilities

• Maintain and share the resident assessment schedule.
• Monitor assessments, applicable assessment protocols and care plan reviews to ensure they are completed on time.
• Coordinate data collection and help the interdisciplinary team translate assessment findings into individualized, resident-centred plans of care.
• Train and coach staff on RAI-MDS,InterRAI LTCF, PointClickCare and Point of Care.
• Identify documentation and coding gaps and provide targeted education to improve accuracy and staff confidence.
• Audit assessment data and Resource Utilization Group (RUG) classifications for accuracy before quarterly submissions.
• Coordinate timely submissions to the Canadian Institute for Health Information (CIHI), review submission reports and manage corrections and resubmissions as needed.
• Analyze clinical and quality indicators, prepare reports and help the team use the findings to guide care decisions, operations and quality improvement.
