Job summary

CarePartners is seeking a Clinical Practice Lead to join its Nursing Team in Ontario. The 20-month temporary full-time role supports the PPRE team, aligns clinical standards, and drives education, training and program initiatives for Ontario Health Teams.

This position involves on-call rotation, evenings and weekends as needed. The role requires RN or RPN licensure with CNO, plus 2+ years of community leadership experience; travel within Ontario may be required.

Qualifications

  • Licensed RN or RPN with CNO
  • Minimum 2+ years leading clinical practice in community
  • Leadership experience is a plus
  • Palliative care experience is an asset
  • Wound care experience is an asset
  • Proficiency in Microsoft Office
  • Valid driver’s license; travel in Ontario may be required
  • Fluent English communication skills; French an asset

Responsibilities

  • Mentor, coach and support front line clinical staff with ongoing clinical education from the PPRE team
  • Collaborate with regional management in Community Health Services and Ontario Health Teams
  • Approve, train and monitor functions per Regulated Health Professions Act and CHS policies
  • Maintain quality management procedures within the Nursing team and department
  • Promote and market Community Health Services; maintain positive public relations
  • Build relationships to enhance local capacity and system navigation supports for holistic care
  • Utilize virtual care tools to support frontline staff and patients
  • Provide active guidance on workplace health and safety
  • Perform other duties as required

Job description

We are currently looking for someone to join our dynamic Nursing Team in the role of Clinical Practice Lead. This person will work closely with our provincial Professional Practice Research & Education (PPRE) team and support clinical standards, orientation, and training. They will collaborate with leaders and help align strategic goals, priorities and initiatives set out and defined by our unique patient populations and Ontario Health Teams (OHTs).
We are looking to fill a Temporary 20-month Full-Time Clinical Practice Lead position in the Waterloo-Wellington - Waterloo Branch.
Participation in the Branch On-Call schedule is required, which includes evenings and weekends on a rotational basis.

  • This position offers a competitive compensation grid, with salary progression based on hours worked and role-specific criteria. The salary range is $79,170.00 to $99,528.00, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.
What The Role Involves
  • Supports, mentors and coaches front line clinical staff while delivering ongoing clinical education designed by the PPRE team.
  • Works with regional management with respect to Community Health Services, in cooperation with the Ontario Health atHome and OHT if applicable.
  • Approves, trains and monitors special functions as outlined in the Regulated Health Professions Act and in compliance with Community Health Services policies and practices
  • Responsible for maintaining quality management procedures and practices within the Nursing team and department.
  • Promotes and markets Community Health Services and maintains positive public relations.
  • Builds relationships to enhance local capacity and understanding of system navigation supports, at a local and regional level, to support holistic patient care delivery
  • Uses new technology . virtual care to support frontline staff and patients to achieve desired care plan goals
  • Promotes and provides active guidance for workplace Health and Safety.
  • Performs other duties as required.
What You Bring
  • Regulated Health Professional Registered Nurse/ Registered Practical Nurse
  • Registered with the CNO.
  • 2+ years of community experience in leading exceptional clinical practice in a community care setting
  • Experience in a formal leadership role considered an asset
  • Strong palliative experience, an asset
  • Wound care experience, an asset
  • Knowledge of adult learning principles, an asset
  • Strong proficiency in all aspects of Microsoft Office.
  • Vehicle is required and some occasional travel within Ontario is required
  • Demonstrated ability to handle highly sensitive and confidential information.
  • Excellent English verbal and written communication skills
  • French language skills are an asset
