Job summary

Canadian Mental Health Association Durham is seeking a dedicated lead for the Community Homes for Opportunity (CHO) program. You will monitor compliance, provide guidance to homeowners, and ensure that CHO operations align with recovery-oriented clinical standards.

Reporting to the CHO Lead, you will coordinate program activities, support clinical staff, maintain documentation, manage waitlists, and collaborate with community partners to connect residents with appropriate services.

Qualifications

  • Post-secondary degree in a relevant field (e.g. Mental Health, Social Sciences) and a minimum of 5 years clinical experience and/or equivalent combination of education, experience, and/or lived/life experiences are required.
  • Sound knowledge of relevant acts and Ontario legislation, including health care consent and social services benefits.
  • Excellent written and verbal communication skills.
  • Ability to develop and maintain positive relationships with clients and partners.

Responsibilities

  • Reviews client intake and assessment information and prepares individualized Client Care Plans focused on long-term stability and growth.
  • Ensures clients understand CMHA Durham services and agrees to service expectations through appropriate agreements.
  • Establishes supportive therapeutic relationships with clients and advocates for and with them.
  • Provides assistance to clients in establishing peer and family support networks.
  • Develops, updates, and monitors compliance for designated homes in collaboration with the CHO Lead.
  • Provides summary compliance reports within approved timelines.
  • Documents contacts, summaries, and reports in CMHA policies and procedures.
  • Monitors reporting system for incidents and complaints with CHO Lead.
  • Provides support and direction to internal clinical staff within the program.
  • Ensures resident care plans align with a recovery-oriented environment and meet clinical needs.
  • Ensures improvements are completed timely and monitors the CHO waitlist.
  • Acts as first point of contact for homeowners and liaises with community resources to ensure continuity of care.
  • Performs other duties as assigned by CHO management.
  • Adheres to client safety programs and infection control procedures in partnership with clients and staff.
