Job summary

AgeCare in Calgary is seeking a Licensed Practical Nurse (LPN) to join our team in a casual, on-site role. You will collaborate with the care team to deliver goal-directed, patient-centered nursing care to residents, including assessments, care planning, and medication administration.

The ideal candidate will have active LPN registration, CPR/First Aid, and experience in senior or long-term care, with strong communication and coordination skills to support high-quality, compassionate care.

Qualifications

• Current LPN registration in the applicable province.
• Experience in senior or long-term care preferred.
• CPR and First Aid certification current.
• Strong clinical knowledge and care coordination.

Responsibilities

• Collaborate with care team using a person-centered approach.
• Conduct resident assessments, care plans, and documentation; communicate updates as needed.
• Implement and evaluate care plans; adjust as required.
• Participate in resident care conferences and support scheduling.
• Assist with resident transitions including move-ins, discharges, and transfers.
• Provide direct nursing care and administer medications.
• Safely manage medications and narcotics in accordance with scope of practice and legislation.
• Educate residents and families about health conditions and care plans.
• Coordinate care plans with physicians, team members, and service providers.
• Delegate tasks to unregulated care providers.
• Respond to significant changes in residents' conditions and manage medical emergencies.
• Oversee emergency response in the absence of senior leadership.
• Support infection prevention, health and safety initiatives, and quality improvement plans.
• Perform other related duties as assigned.
