Qualifications

• Registered Social Worker in good standing with BC social work regulatory body.
• Experience in home health or community-based settings preferred.
• Strong counseling, communication, and interpersonal skills.

Responsibilities

• Provide comprehensive assessment, care planning, and treatment coordination for designated clients and families.
• Collaborate with multidisciplinary team to deliver psychosocial care and group work.
• Support system navigation, advocacy, and connecting clients to specialized resources.
• Deliver education to clients and families and promote client self-management.
