Job summary

Interior Health Authority is seeking an experienced Community Health Worker for Lillooet Hospital and Health Centre in Lillooet, BC. You will support home-based care, assist clients with daily living activities, and work with a team of nurses and LPNs to promote independence and wellbeing.

The position is permanent part time (0.98) with day and evening shifts, contributing to a dynamic home health program in the Fraser Canyon area.

Qualifications

• Community Health Workers provide home support services to clients, assisting with personal care and daily living, while promoting maximum independence.
• They work as part of Nurses and LPNs to support client independence and community care plans.
• Experience in community health or home care is a plus.

Responsibilities

• Assist with activities of daily living (bathing, dressing, grooming).
• Perform household tasks to maintain a safe home environment.
• Prepare meals according to care plans and dietary requirements.
• Assist with mobility and transfers.
• Observe, document, and report changes in client condition.

About The Job

As a Community Health Worker, you will work as part of a dynamic team of Nurses and LPNs who support Community Clients’ independence and ability to stay at home. Community Health Workers provide home support services to clients, by assisting with personal care and daily living, while promoting maximum independence. Community Health Workers are key players in the home health care plan for each client.

Typical Duties Include

• Activities of daily personal care, such as bathing, dressing, grooming and oral hygiene
• Household duties related to maintaining a safe and healthy environment in the home
• Meal preparation in accordance with designated plans
• Assistance with mobility and transfers
• Observing and reporting changes in client behaviour and condition
• Performing delegated tasks according with training and procedures

This is a Permanent Part Time (0.98) opportunity.

Shifts of work will be scheduled days and evenings rotating from 07:00 to 15:30, 13:00 to 21:30 hours.
