Providence Care is seeking a Registered Practical Nurse (RPN) at Providence Transitional Care Centre in Kingston to deliver safe, patient-centred care in a non-acute setting under the Standards of Nursing Practice of the College of Nurses of Ontario.

This full-time role requires an active Ontario license, current CPR, and relevant certificates; you will support patients and caregivers, collaborate with an inter‑professional team, and contribute to quality of life during transitions while

Qualifications

• Active, unrestricted license to practice with the College of Nurses of Ontario.
• Current CPR certificate.
• College certificate required, diploma preferred.
• Medication Certificate dated after 1985 which meets the requirements of the Ontario College of Nurses.
• Nurses, if graduated before 1985.

Responsibilities

• Demonstrates competencies and skills essential for safe and effective patient care.
• Assists in evaluating health status of patients with complex care needs and provides direct care.
• Shares in establishing priorities for care including preventive, therapeutic and rehabilitative aspects.
• Advocates for patients and their caregivers, recognizing them as major decision makers.
• Seeks assistance when knowledge or skill is insufficient or beyond scope of practice.
• Evaluates outcomes of patient care plan and modifies the plan based on patient responses.
