Cogir Senior Living in Milton, Ontario is seeking a Registered Practical Nurse to join our care team. You will provide planned, patient-centred nursing care to residents, in collaboration with a multidisciplinary team, following provincial standards and care plans.

Qualifications

• Current registration as a Registered Practical Nurse in the applicable province.
• Demonstrated sound clinical knowledge.
• Experience working with seniors and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, community/long term care an asset.
• Strong drive, respect and an understanding of the emotional and physical demands of the role.
• Focus on teamwork and customer service with the ability to build trusted relationships.
• Strong written and oral communication skills and critical thinking skills.
• Demonstrated organizational and time management skills.
• Computer proficiency and experience with Microsoft Office applications.
• Current Basic Cardiac Life Support and First Aid certification required.
• Current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one

Responsibilities

• Provide planned, patient-oriented nursing care in accordance with provincial standards of care.
• Be an integral part of our multi-disciplinary team who build trusted relationships with residents, families and teams with continuous ongoing and effective communication.
• Implement and evaluate the effectiveness of the care plans and make changes as required.
• Complete and ensure resident's clinical care assessments, care plans and documentation are accurate, up to date and communicate changes as required.
• Teach, delegate and direct unregulated care providers, providing them with coaching & mentorship.
