Job summary

ParaMed is seeking Registered Nurse (RN) or Registered Practical Nurse (RPN) to provide home-based care across a Canadian service area. You will visit patients in homes, assess needs, implement and monitor care plans, and document care interactions.

The role supports autonomy, ongoing learning, and strong teamwork with supervisors and peers. Applicants must hold a CNO license, have a valid driver’s license, and be willing to travel and cover after-hours on-call.

Qualifications

  • Active nursing license with CNO in good standing.
  • Valid driver’s license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Willingness to travel within a designated area and on-call coverage.

Responsibilities

  • Assess patients using appropriate tools to identify individualized needs.
  • Execute and monitor care plans and adjust as needed.
  • Engage patients through rapport-building and collaborative care.
  • Maintain and update professional patient records from encounters and plans.
